24/7 Supervision & Support
Awake staff are available around the clock to promote safety, comfort, and assistance when needed.
Personal Care Assistance
Support may be provided with personal hygiene, grooming, dressing, bathing, and other activities of daily living.
Wound Care Support
Wound dressing changes and related support may be provided when appropriate and within the home's scope of services and applicable requirements.
Behavioral & Mental Health Support
We provide a supportive environment for adults experiencing mild to moderate mental health challenges while coordinating with appropriate healthcare and behavioral health professionals.
Community Engagement
Residents are encouraged to participate in activities and community opportunities based on their interests and abilities.
Medication Support
Medication reminders and medication administration are provided in accordance with applicable Wisconsin requirements and each resident's authorized care needs.
Diabetes Support
Residents who require diabetes-related assistance may receive support based on their individual care needs and orders.
G-Tube Support
G-tube site care and feeding assistance may be accommodated based on individual needs, orders, and regulatory requirements.
Care Coordination
We work collaboratively with families, guardians, case managers, managed care organizations, healthcare providers, therapists, psychiatrists, and other members of the resident's support team.
Individualized Support Plans
We work with each resident and their support team to understand their preferences, needs, strengths, and goals.
