Review & Assessment

Our team reviews the information to determine whether Vital Care Comfort Home can safely and appropriately meet the individual's needs.
Tour & Conversation

Prospective residents, families, guardians, and referral partners are encouraged to tour the home and ask questions.
Care Planning

If the home is an appropriate placement, we work with the resident and their support team to establish individualized services and expectations.
Admission

Once required documentation, agreements, funding arrangements, and other admission requirements are completed, the resident can transition into the home.
